If you see your surname twice or there is a typo in your name, this means your name is stored incorrectly in our system.
When you use the below form, it updates the name we have on file.
After updating your name, you will be taken to the log out page.
You will need to log out and then log back in again to refresh the system and for the new name to take effect.
